I understand that this Mindfulness for Fertility (MFF) Program includes skill training in meditation techniques. I understand that if for any reason I am unable to, or think it unwise to engage in these techniques and exercises either during the weekly classes or at home, I am under no obligation to participate in them. I understand that I am committing to attend each of the six (6) weekly sessions, and to practice the home assignments during the duration of the training program. This agreement releases the instructors (Stephanie Curran & Alda Ngo) from any liability relating to potential injuries that may occur due to participation in the Program. I also acknowledge the risks involved in the MFF Program.These may include but are not limited to physical injury and emotional distress. I swear that I am participating voluntarily. I will also make every effort to observe safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.
